Keep observations, quotations and evaluations in separate fields. A transcript can support note verification, but transcription errors and speaker mistakes still need review. Do not let a summariser invent a competency score or a negative inference from an unclear answer. A structured manual note template remains a valid option. Before the interview, confirm the permitted recording or note-taking method and explain it to participants. During the interview, use the agreed questions and record evidence against them. Afterward, check names, figures and important claims against the authorised record. Label uncertain passages and remove unsupported conclusions. The interviewer owns the final assessment.
